A recognised treatment that can fit into your daily life

Mandibular advancement devices (MADs) are among the recommended treatments for snoring and obstructive sleep apnea. In patients for whom they are suitable, they can reduce apneas, snoring and sleepiness, and improve quality of life.3

They may be considered in particular when CPAP is poorly tolerated or another option is preferred. The choice depends on your medical assessment, your dental situation and your expectations.

CPAP remains the preferred treatment for severe sleep apnea. The Panthera instructions for use provide for second-line use after refusal, failure or intolerance of CPAP, in line with applicable national recommendations. The decision requires an individual medical assessment and a check of the result.

How does the appliance work?

It holds the lower jaw in a forward position to help keep the airway open during sleep. The advancement is adjusted gradually according to your comfort and your response to treatment.

A slim, lightweight design

The low bulk in the mouth and the space left for the tongue are designed to make the appliance easier to wear at night.

A durable, metal-free medical material

Polyamide 12 is a biocompatible medical-grade nylon, light and strong. The metal-free design and simple cleaning make daily use easier.

Custom digital manufacturing

Digital impressions are used to design your appliance, which is then 3D-printed. With more than 300 customisation options, its configuration is adapted to your dental situation.

Fine, gradual adjustments

Mandibular advancement can be adjusted over the course of treatment to find the right balance between effectiveness and comfort.

Care led by Dr Chira Riahi-Boukhris

Head of oral appliance therapy at Shalvina
Member of the American Academy of Dental Sleep Medicine.

Assessing your situation and planning treatment

Your doctor’s letter recommending the appliance and your test results are reviewed. Examination of the teeth, gums and jaw joints establishes whether treatment is feasible. The expected benefits, precautions and terms of care are explained to you.

Taking your digital impressions

If the treatment is chosen and your situation allows, impressions are taken at the first appointment and sent to the manufacturer the same day.

Supporting you at fitting

Your appliance is delivered with a personalised starting position. Dr Riahi-Boukhris checks the fit and explains how to wear, handle and clean it.

Adjusting treatment and monitoring progress

In-person follow-up visits are planned at around one month and six months after fitting, with a schedule adapted to your progress. Appointments are booked in advance. The Shalvina team supports your adaptation with gradual adjustments tailored to your comfort and to the treatment result, while monitoring your dental health.

Checking effectiveness is part of follow-up

For sleep apnea, a follow-up sleep test with the appliance in place is used to assess the treatment result.3

The results are reviewed with you, in coordination with your doctor, to guide the next steps.

Where relevant, nasal breathing, weight and sleep position are also taken into account as part of comprehensive care.

Your questions

My doctor has already referred me for an oral appliance: can I book directly?

Yes. You can book the dental assessment directly with Dr Riahi-Boukhris, without a further routine consultation with Dr Marc Boukhris.

If your choice of treatment still needs a medical assessment, you can see your own doctor or book our snoring and sleep apnea consultation.

Is the appliance comfortable?

Its slim profile, light weight and custom fit are designed to make it easier to wear. An adaptation period may be needed. The Shalvina team supports you with gradual, personalised adjustments that take account of your comfort and the treatment result.

How can I tell whether my appliance is working?

Less snoring and better sleep are useful signs, but they are not enough to confirm that sleep apnea is controlled. A sleep test with the appliance in place assesses its effectiveness, after the adaptation and adjustment phase. Its timing depends on your situation. The results are reviewed with you, in coordination with your doctor, to guide the next steps.

How long does it take?

About two weeks from the impressions to the fitting of your appliance (indicative timeframe).

What does the package include?

The package includes the initial dental assessment, digital impressions, the appliance, fitting and adaptation, adjustments and in-person follow-up visits at around one month and six months after fitting. The schedule is adapted to your progress. Fees and any treatment not included are explained before you commit.

Can private insurance contribute to the cost?

Some private insurers may contribute to the cost of a mandibular advancement device. This depends on your policy and your insurer’s terms. Check your cover and any required steps with your insurer before starting treatment.

What warranty does my appliance have?

Your Panthera appliance has a three-year manufacturer’s warranty covering breakage of the device, under the manufacturer’s terms. Loss and damage caused by pets are not covered. Repair or replacement arrangements are explained before treatment begins.

What is the relationship between Shalvina and Panthera?

Shalvina’s founders are also involved in distributing Panthera devices in Israel. The choice of treatment is based on an individual medical and dental assessment.
